Effective treatment for people living with chronic conditions like hypertension must be consistent and accessible. Patients shouldn’t have to travel long distances to specialized hospitals for care. When they are forced to do so, they are more likely to miss medical visits, stop taking their medications, and risk life-threatening consequences.

Experts from Resolve to Save Lives’ India team, in close collaboration with other partners of the India Hypertension Control Initiative (IHCI) demonstrated the transformative impact of decentralizing care delivery for hypertension to community-based primary care facilities. This multicenter study, conducted across 4,045 public health facilities and involving 394,038 people living with hypertension, showed that decentralized care significantly addressed patient retention and improved blood pressure control from 20% to 58% in less than five years.
